The successful applicant will be an NHS employee and may therefore have contact with vulnerable service users. We have an exciting opportunity for an experienced, caring and motivated Community Nurse Team Leader within the Probus and Roseland Community Nursing Team. This will be an evolving job role within the Primary Care Network exploring multi-disciplinary ways of integrated working. Therefore flexibility and a willingness to explore new ways of working is a must.
We are seeking a dynamic, forward thinking professional who wants to help us deliver and coordinate exceptional community nursing care, and provide leadership to the team.
Our patients are at the centre of all that we do so we are looking for a team leader with an interest in delivering the highest quality care and to support our patients to lead full and independent lives whilst promoting self management of their health conditions.
A commitment to team working and providing excellence in patient care is a must as are exceptional communication and people skills.
We are looking for a personable colleague who is willing to collaborate with our wider health care team across the County and who has an understanding of the changing climate of care delivery in the community including ‘Aging Well’ (NHS Long Term Plan) and Urgent Care Response.
You will need to be able to demonstrate real enthusiasm for community nursing as well as a desire for on-going professional development.
