FULL TIME HOSPICE RN CASE MANAGER

We are seeking a highly motivated, compassionate and energetic Registered Nurse. This Hospice RN establishes a professional and caring relationship with our patients and families, support staff, physicians, discharge planners, social workers, facilities and other health care professionals by providing hands-on care, on-going clinical management and education regarding hospice care. Job Requirements

Qualifications:

• Registered Nurse with a current RN license
• Current vehicle liability insurance
• Bilingual preferred, not required
• CHPN certification a plus
• Excellent people, verbal and written communication skills
• Ability to think on your feet
