Consumer Director Services is seeking an Intake Coordinator to add to our team!
What does our company do? CDS in Texas provides a foundation of support that enables individuals with disabilities to self-direct their care.

Essential tasks:

Accepts or assists with intake processes including inquiries, referrals, questions, and complaints or concerns directed at Intake.
Verifies client payor and eligibility for services and enters new client information into agency data base and distributes correspondence as required by process.
Processes referrals; completes referrals; sends fax documents as requested; collates forms; and copying of documents.
Verifies names and addresses of potential clients; maintains referral log of admitted and non-admitted clients.
Documents on Communication Notes all contact with patients and staff concerning patient care or issues.
Locates and communicates physician orders as needed.
Complete referral form on each patient.
Coordinators services with affected offices and departments as required by process; maintains activity records as required by process.
Complies and analyzes data as directed.
Documents any delays in client treatment or services with Medicaid, Medicare, and private insurance companies payors; notifies the appropriate personnel for follow-up when changes in client’s eligibility status occur.
Maintain good communication between all departments.
Cross-train with other members of the department.
Observe confidentiality policies at all times.
Comply with federal, state, & company policies, procedures, and regulations.
Perform other related duties as assigned.

Requirements:

Must be at least 18 years of age, with high school diploma or GED, required.
Two (2) to three (3) years of call center customer service and/or health care setting experience.
Bilingual (Spanish) preferred.
Able to work independently.
Able to work with a team to accomplish goals and objectives.
Excellent time management skills.
Effective organizational skills with strong attention to detail.
Excellent interpersonal relations skills, including active listening skills such as giving full attention to what other people are saying, taking time to understand the points being made, and asking questions as appropriate.
Written and verbal communication skills are required to communicate effectively as appropriate to the needs of the audience.
