The Journey Home is an 18-bed intensive Group Home for male identifying clients ages 12–18. Youth at The Journey Home have complex trauma, mental health, medical, cognitive, and behavioral challenges. The Journey Home works to help adolescents integrate into the community in a supportive way through a strengths based, permanency focused treatment model. Within the safety and structure of a home-like environment and community, The Journey Home clients are able to build a confident sense of self, self-regulation skills, and a support network of positive relationships to move forward from The Journey Home with a sense of mastery and autonomy.

Responsibilities

The Permanency Clinician is responsible for managing a designated case load of clients. The essential function of this position is to deliver the Permanency Practice Model for each client on a caseload. The provision of permanency services will be delivered through Family Search and Engagement, Permanency Readiness activities and Youth Guided-Family Centered Teaming to expand each individuals support network. The mission is to develop safe, healthy and long-lasting relationships for clients who have little or no support systems to foster and establish permanent care giving relationships. Permanency clinician will assist in leading the permanency model at LA by providing training and consultation to program staff. The Permanency Clinician will provide family connections support and therapy. Training in the model will be provided.

Requirements:

Minimum of Master’s degree in Social Work, psychology, counseling or related field from an accredited college or university preferred.

Congregate care or mental health experience preferred.

Experience working with specialized populations of children and adolescents preferred.

Experience working with children and families involved with the child welfare system in a permanency related context.

Experience in family engagement/work preferred.

Knowledgeable in growth and development, childhood trauma and permanency; able to obtain and interpret information in terms of the needs of clients.

Knowledgeable regarding DCF regulations, policies and practice.

Acceptable CORI (Criminal Offender Record Information) and OIG background check

Maintains a valid driver’s license

Visual and hearing acuity to perform job related functions

Bilingual bonus – Bilingual with the ability to read, write and speak in a language other than English proficiently. $500 after 3 months of successful employment, $500 after 6 months of successful employment.
